A survey will be conducted on the sports activities of Tokyo residents, targeting 4,000 people randomly selected from the Basic Resident Registers managed by wards, cities, towns, and villages.

Those who are requested to participate in the survey will be mailed a request letter, questionnaire, etc. in advance (approximately two days before the survey begins) (surveyors will not visit your home).

Please rest assured that survey responses will be anonymous and will be used only for the purpose of this survey. Furthermore, the Tokyo Metropolitan Government and the survey implementation organization have taken strict measures to protect personal information.

1. Please return the completed survey form in the enclosed return envelope (no postage required), or
2. Please respond online using your computer or smartphone.
